Usually people are applying for the preliminary IM to go to advanced specialty after that: Anesthesia, Radiology, Neurology, etc.
This year does count if you want to continue doing IM, so you will start with a PGY-2 position.

Transitional is somewhat similar but is mostly forstudents who have not made their mind yet whwre would they want to go. It is very similar to preliminary IM as it satisfies requirements for most advanced programs. On top of that transitional year is much more relaxed than prelim IM.
